By accessing our website or engaging our services, you agree to these Terms in full. If you do not agree, please discontinue use of our website and services.
1. No Doctor-Patient Relationship
Use of this website, submission of a contact form, or booking a consultation does not establish a doctor-patient relationship. A formal physician-patient relationship is only established after an in-person consultation with one of our physicians, completion of intake documentation, and mutual agreement to proceed with a treatment plan.

2. Informational Purpose of Website Content

The Forest Lounge provides physician-supervised health optimization services including hormone therapy, medical weight loss, peptide therapy, cognitive performance programs, and Direct Primary Care membership. We make no guarantees, representations, or warranties regarding specific treatment outcomes. Individual results vary. Past results described in any context — including testimonials, case studies, or service descriptions — are not a guarantee of future results for any patient.
4. Prescription Medications and Controlled Substances
Certain services offered by The Forest Lounge involve physician-prescribed medications including but not limited to hormone therapies and GLP-1 receptor agonists. Prescriptions are issued solely at the discretion of our physicians based on clinical evaluation and laboratory results. Submission of an inquiry, completion of a consultation, or payment for any service does not guarantee that a prescription will be issued. Our physicians reserve the right to decline to prescribe any medication for any reason consistent with their professional and ethical obligations.
5. Booking, Cancellation, and No-Show Policy
Consultations We require a minimum of 24 hours notice to cancel or reschedule a consultation appointment. Cancellations made with less than 24 hours notice may be subject to a cancellation fee.
No-shows Failure to appear for a scheduled appointment without prior notice may result in a no-show fee and may affect your ability to schedule future appointments.
Fees Current cancellation and no-show fees are communicated at the time of booking and may be updated periodically. Booking a consultation constitutes agreement to our cancellation policy in effect at the time of your appointment.
6. Payment Terms
Payment for services is due at the time of service unless otherwise agreed in writing. Membership fees are billed on a recurring monthly basis as described at the time of enrollment. All fees are non-refundable unless otherwise stated in writing by The Forest Lounge.
We do not accept insurance as direct payment for most services. Patients are responsible for all fees regardless of insurance coverage. We will provide documentation to support insurance reimbursement claims where applicable, but we make no guarantee of reimbursement.
Pricing for services including medical weight loss programs, hormone therapy, and membership is subject to change. Current pricing is communicated at the time of consultation and may be updated at our discretion.
7. Membership Terms
Direct Primary Care membership at The Forest Lounge is a monthly membership arrangement and does not constitute health insurance. Membership provides access to primary care services as described at enrollment and does not cover hospitalization, specialist care, imaging, laboratory services beyond those included in your plan, or emergency care.
Membership may be cancelled with 30 days written notice. Members are responsible for fees accrued through the end of the notice period. The Forest Lounge reserves the right to terminate membership at any time with 30 days notice.
8. Laboratory and Diagnostic Services
Laboratory testing ordered by our physicians may be performed by third-party laboratories. Fees for laboratory services may be separate from consultation or program fees and will be communicated in advance where possible. The Forest Lounge is not responsible for errors, delays, or results from third-party laboratory providers.
9. Telehealth
Where telehealth services are offered, they are subject to applicable Illinois state laws governing the practice of telemedicine. Telehealth services may not be appropriate for all patients or conditions. Our physicians reserve the right to require in-person evaluation at any time.
10. Intellectual Property
All content on this website including text, copy, graphics, logos, images, and service descriptions is the property of The Forest Lounge and is protected by applicable intellectual property laws. You may not reproduce, distribute, modify, or create derivative works from any content on this site without our express written permission.
